We at Dr Maria E. Cruz, DDS Dental Practice are proud to be part of a team whose primary goal is to deliver the most excellent and most comprehensive dental care available today. We are concerned about your dental health and want to ensure that it is attended to responsibly. With much research and consideration, we are pleased to offer the following plans to help our patients receive optimum and affordable dental care.
Our Office Policies Regarding Fees
INSURANCE
Insurance is a great benefit for our patients and we are pleased to file directly to your insurance company under the following guidelines:
a. We ask that you bring your insurance card and completed dental form on your initial visit. A copy of your company's benefit booklet would also be helpful.
b. Estimated co-payment is to be paid on the day of your visit.
c. Most insurance companies will pay within 30 days. If your insurance company does not pay within 30 days, the balance becomes your responsibility and is due at that time.
d. Not all services are covered benefits in all contracts. Our staff is well trained for remitting in optimum coverage and payment, however, some procedures are not covered with some plans. We will try to advise, but due to multiple plans with different coverage, it is in your best interest to find out all of your insurance benefits for maximum dental coverage and usage.
NON-INSURANCE
We offer more creative approaches to help you receive needed dental care.
a. For Senior Citizens (65 and up), a 10% discount is extended if payment is made by cash or check only and if payment is made on the date dental care is provided.
b. Visa, Mastercard, Discover and American Express are accepted.
c. CareCredit - Another convenient and affordable way to make monthly payments on your dental treatment. No interest options. 6, 12, 18 & 24 month plans. (Verification of credit standing may be necessary).
Statements
To help reduce our costs and thus pass the saving to you through our fees and services, we ask that billing be unnecessary. When necessary, accounts 30 days past due will accrue at 1.5% interest per month. All dental services are rendered in good faith and we ask that collection never be necessary. We want to keep our dental relationship for a long time!! Q. Do you file dental insurance?
A. We will gladly file your insurance as an out-of-network provider. Please prepare to pay your co-pay or deductible when you visit. We accept assignment of benefits, which means that your insurance provider can reimburse us directly for their portion of your balance. If your carrier does not reimburse us the expected amount, we will adjust your balance accordingly. For answers to your questions about dental insurance please call us.
Q. Does my insurance cover that?
A. Dental insurance is diverse. Some policies cover only a small percentage of fees, while others cover 50 percent or greater. The more you or your employer pays for premiums, the better the coverage, in most cases. Generally speaking, dental insurance covers some restorative and preventive procedures. Cosmetic treatment is not usually covered. We offer Care Credit financing to make cosmetic dentistry affordable for all patients. If you have a question, give us a call. We'll be happy to tell you what your insurance will cover.
Q. What can I expect my first visit?
A. Your first cleaning appointment you can expect to be here for 1 hour. Please arrive 10 minutes before your appointed time. You will be scheduled with one of our 3 qualified dental hygienists, she will go over your health/dental history, take "cavity detecting" radiographs, perform a through cleaning and have Dr. Maria E. Cruz, DDS do a comprehensive exam. We will review your treatment plan, discuss your dental insurance, and help make it possible for you to move forward with your treatment by trying to work out a reasonable payment plan.
Q. Do you do Cosmetic dentistry?
A. Dr Maria E. Cruz, DDS can repair teeth, straighten teeth, whiten teeth and even replace teeth to enhance appearance. We offer bonding, crowns and bridges, and tooth whitening.
